Service was terrible. They were out of all drinks except tea, water and sprite and the tea tastes old. Menu lists beer but they said they no longer have a liquor license. Waitress was rude. Employee blasted the juke box so couldn't hold a conversation. Food took forever. Bread sticks were awesome. Pizza was mostly sauce. Bunch of kids running around without adults screaming and horsing around.

Service was top-notch. The pizza was good. Lots of fresh toppings, especially the mushrooms. The sauce was sweet, which I'm not a fan of. Overall, it's pretty good

This evening my husband took our daughter and her friend to Pizza Forum for dinner, it was a win because we had coupons to use on Pizza & Bread sticks. It was her friends very first visit. Her words were, “wow this is the nicest restaurant I’ve ever been to.” It was our 2nd visit, actually was better than the first visit.
We ended up ordering the girls a cheese pizza and a pepperoni pizza plus an order of bread sticks. The girls ate a couple bread sticks and 4 slices of pizza each. They loved the pizza. The bread sticks were loaded in butter and garlic sauce. My husband and I ordered an XL pepperoni and mushroom pizza. It was so delicious. Yes, we have plenty of leftovers for tomorrow’s lunch or dinner. Cold pizza is the best ever.
The restaurant was nice and clean. Bathrooms were also clean, that’s a plus in my book. Nasty bathrooms make for a bad experience. Our server was great, altho it took her a bit to refill my drink, all is good she got to it. Over all we had a pleasant experience this evening. They played Christmas Music in the background. That was nice for the Holiday. Very enjoyable to listen to. We will definitely be back to Pizza Forum.

Ordered delivery and 20 minutes after when it was to be delivered called. The man who answered phone just said it was there and the girl was out delivering and he could not tell us when it would be delivered . When I asked if it would still be warm he said it would be hot. Decided to just go pick it up . By the time we got home it was not hot, cheese was nasty, breadsticks were Leaking all over my car and were cold. We threw it away and wasted our money. Poor service and food . Never apologized or even seem to care when we picked it up.

Great pizza, the Hawaiian is a hit with the kids. We had the meatballs as an appetizer and it was amazing. I don't like leftovers but took home the remaining meatball and ate it the next day.

Genuinely will not be back here. Ordered take out, got breadsticks and a small pizza, and two 32 oz drinks.
Those are absolutely not 32 oz drinks. That is a small drink at McDonald’s?. For the price, that is not worth it.
The grease from the breadsticks leaked through STYROFOAM and into the bag, and it created an actual pool of grease that also leaked through the bag.
I usually don’t write bad reviews, but this was really irritating especially since I left a 3 dollar tip, and it was 23 dollars before that as well.

This place has great pizza! I don't know about service. I went during the fair so they were outside selling pizza too. Rhat might be why it took forever to get service. Took quite a while foe pizza to get to the table too. They had games to alleviate the wait time. Overall giving them a pass on service due to how phenomenal the pizza was! They have a rocket game that seems cool but it isn't it just goes back and forth you can't control it just so you know.
Kid-friendliness: Them have games to play but they are old so watch out! One of them stole our money.
