If you’re looking for Italian style tender pizza crust and top notch service, this is the place. It’s Vernal, ok, so don’t expect awesome decor. Maybe someday they’ll get there. But for where we’re at, and for what this is, I’m impressed. Expect to pay about $25+ per person. Nearly everything is handmade and done well. Take a look at their specials for a good (and tasty) deal.

WOW!!!! Absolutely the MOST DELICIOUS AUTHENTIC amazing food in all of my travels this far!! Between the house made Mozerella cheese, fresh homemade pizza crust with fabulous flavor, Pizza sauce is perfect, homemade soup was scrumptious I can’t decide what I loved most! Even the salad was out of this world! Service was also top notch! Fast friendly attentive service! And the atmosphere is cozy & nice. Love this place!!! Bravo!

The staff at Antica Forma is amazing - when I had to jump in to help someone with arranging a quick meal they were on it! Shrimp Alfredo to the rescue - it was a great experiance for everyone involved and was a perfect example of going above and beyond. Thank you for an amazing meal yesterday! The bruschetta is the perfect appetizer- fresh tomatoes and just enough to share but not fill up on, and the pizza and garlic bread were delicious! Looking forward to our next meal there!

Waitress was constantly behind and it was not that busy. Prices are on the high side. Food is small portioned and slightly expensive.
Great dinner in basin. Their bread sticks are great, ask for some dipping sauce.
The Fried Calamar appetizer was perfect..Clam dish was SPOT ON !! The shrimp, mussels, calamari dish was a bit of a disappointment. The calamari was OVER COOKED. Being a chef, I was a bit let down, but I'm sure it was just an off day . I WANT TO GO BACK & taste some of the pizza.

My husband built his own pizza oven in 2020 and makes his own dough. He loved this place and wished they had one in Salt Lake so he could learn from them ;). I also thought the pizza was so delicious! The price for the food was well worth it.
