A formal complaint about unacceptable food safety practices to DC Department of Health Division of Food and the Chipotle Mexican Grill corporate headquarters.
This complaint includes rude manager behavior, poor food safety actions by a line worker, and unsanitary conditions observed at the Chipotle located at 1045 5TH ST NW WASHINGTON, DC 20001: Ward 6 | Quad: NW. The incident occurred on the 20th of October, 2025. The facts below require immediate investigation, corrective action.
Incident Details
• While waiting in the service line, I observed a line worker repeatedly violate basic food safety protocols. The employee used a gloved hand to wipe food from the metal counter and push the debris onto the floor, then continued assembling the next order without changing gloves.
• The same employee reached into a bag of soft tortillas with that same gloved hand.
• While assembling a bowl, guacamole fell onto the metal counter. The employee used the same gloved hand that had just contacted the dirty counter and the bagged tortillas to push the guacamole back onto the food in the bowl.
• When another customer raised a concern about these clearly unsanitary practices, the manager on duty, identified as Taliah Swann, responded by yelling at the customer for placing a hand on the glass separator instead of addressing the food safety concerns.
• The manager refused to investigate or discuss the customer’s concern, denied service to the customer, and instructed the customer to leave the restaurant without attempting to resolve the issue.
Violations and Concerns
Direct cross-contamination risk from handling ready-to-eat food with gloves that had contacted contaminated surfaces and a bagged product.
Failure to follow basic glove-change and surface sanitation procedures required in any food-service environment.
Managerial failure to supervise and correct staff food safety violations.
Rude, hostile treatment of a customer who raised a legitimate public-health concern.
Overall lack of cleanliness and apparent disregard for food-safety protocols at the establishment.
Requested Actions
Conduct a thorough investigation into the events described, including reviewing any available video, interviewing the employees involved, and documenting corrective actions taken.
Provide immediate retraining for all staff at this location on glove use, cross-contamination prevention, and proper handling of ready-to-eat foods.
Review managerial conduct and, if warranted, take disciplinary or corrective action regarding the conduct of the manager on duty, Taliah Swann.
Inspect and verify that this location is complying with local health code standards and corporate food-safety policies.

CLOSE THIS DOWN that’s all i can say THE worst chipotle in Dc. The service is awful. The food is either cold or overcooked all the time I’ve been coming to the chipotle for the last five years. The last two years has been horrible. The management is rude and never smells clean in there compared to the rest of the chipotles in the city. The one is barely getting by corporate needs to handle this immediately.

Today me and my son visited this location and there was a manager in the lobby having lunch she was having a loud conversation with her crew asking one of the crew members to translate for her that the rice was mushy. Oh wow I'm trying to order me and my son's food it didn't dawn on me that the rice she was speaking of was in the line being served to other customers. As I got ready to pay for my food. The cashier asked me would there be a anything else I would like I asked for water cups she indicated that there was no ice. I was ok with that. Once me and my son got to our destination to try the food we noticed that it was Grease pouring out of the bottom of the bowl and it had completely soaked the bowl causing the bowl to unstable and soft. We both began to eat the food and notice even more harsh conditions about the food. We will not be visiting this location again. I wish the manager would have made the crew pull the rice off the line and make fresh rice the correct way.

The employees have nothing to do with professionalism, there is no help, they are rude, I wanted to wash my hands in the sink, they say you have to pay first!!

Friendly staff, tasty food, easy in-and-out ordering or pickup, if the burrito is too big the line staff outsource it to the experienced Hispanic staff member who looks at you and their coworker with great discontent over the size of said burrito and wonders how I am even going to eat this in one sitting, clean bathrooms, the soda machine works…
11/10 recommend.
