Since you all have been dodging my calls since November I'm gonna go ahead and take it to a public forum. See if that catches your attention. After I quit I called in October to update my address. I called again in November to make sure it was right. When I didn't receive a W2 at the beginning of the year I called again. Since then I've called several times. Somehow there is never a single manager in the building? Not a single phone call or email response. The W2 has STILL not been issued. Unsurprising given how the restaurant is run. I know you have my phone number and email. If I haven't received a W2 by end of day tomorrow I'll be filing IRS and BBB complaints. Have a blessed weekend.

We visited Surv for the first time last night and I was a bit disappointed. We ordered our drinks and it took about 25 minutes to get our first round to the point where we ordered our second round when the first round came out. They were very slow in general (it was a Saturday night). Eventually we got to order our food but they were so far behind that she took our whole order at once, both apps and meals. The good news is that the food was good. We got the mushroom spring rolls and tuna tartar which were both excellent. The pizza was pretty good but I thought the dough was understated. I got the short rib as my meal which was delicious. My only strong complaint about the food was the mashed potatoes that came with the short rid. Over salted and had a strong garlic flavor that wasn’t pleasant. I didn’t even eat them. Overall the food was good but the service and drink situation was very underwhelming.

Major disappointment. It took 25 minutes to order drinks, then took 20 minutes for the drinks to get to the table, by the time it arrived, the $18 shot of whiskey was diluted by the already half melted king cube that was left. The second round took so long I finally went to the bar, in which they were able to get me a drink right away. The burger was severely overcooked, had three bites, I informed the server it was not good, but she didn’t care enough to take it off the tab or make it right in any way shape or form. Would not recommend this place at all.

I love bringing my girlfriends here when they recommend going out to grab a bite!! Kayla and Bryan were our servers and they were top notch - super attentive asking us how we were enjoying our meal throughout the evening, giving recommendations for everything from cocktails to dessert! Beautiful atmosphere, amazingly plated fresh food cooked to perfection with generous portions!! You all hit it out of the park every time!!! Thank you!!

It was terrible. No enjoyment at this place. I’ve went to fine dining restaurants and this is not one. I believe the owner needs to learn to treat her staff as well. Nothing good to say.

I've had the pleasure of dining here a few times & have no complaints! This restaurant is great for mushroom lovers & has options for those like me with dietary restrictions. Perfect place for a date night or to celebrate. Awesome selection of mixed drinks, beer, and wine. Their pizza is delectable as well.Wheelchair accessibility: Ramp access

We very much enjoyed dining here. The booths were comfortable and the service was great! We were in and out in an hour, and it was the Saturday evening dinner rush. The food was very delicious. The only odd thing was the “magician” person going from table to table doing card tricks. We were grateful our check came before he got to ours. All in all, we were impressed and will recommend to others!

We went on a Friday night for our wedding anniversary. Wasn't too busy. Service was ok, it did take a while to get our food. We felt rushed to order. The ambiance was classy, high end. The food was delicious. We had the Charred Brussels sprouts as the appetizer or medium plate and the Deconstructed Tuna Lettuce Wraps as the main dish or large plate. Also had desert, crème Brulee, and drinks. The major complaint was the menu. It was a bit confusing. I suggest if you are unsure on what you are ordering, ask. It's bit pricey. It's worth going for a special occasion.

The server and food were excellent. We had the chicken waffles, braised short ribs, pizza, Brussels sprouts, and chili. All was excellent! The magician Jesse was awesome, very entertaining! We will be back with the kids next time.

Everything was excellent!. Jen was a wonderful waitress . The food was delicious. The manager came by to make sure we were happy with everything. Thank you for another great meal and a lovely evening!

We tried this restaurant for the first time on Valentine's Day. We were apprehensive because of reviews we had read but, we enjoyed Surv! The food came out fast and was cooked perfectly. The ambiance is “different”, since it's a large space...It's a little loud given some of the seating however, the staff made adjustments for me since I have trouble with background noise. Very pleased, I'm sure we'll visit again soon!

Went specifically for mushroom pizza and was so disappointed. The crust had zero flavor like cardboard. Toppings were done in unappealing way. Did not look to be cooked in fire oven. Did not bring leftovers home. I will say others in group liked their choices burgers, tacos, salad and egg rolls. Mocktail was delicious!
