Charming, tiny, great food and service. Pastas were good. Cod caponata - yum. Really good coffee.
Very unique and interesting place. Small and cozy, the owner and chef and his wife were so welcoming. LOVED IT
Incredible experience! The food was amazing and the staff were all so sweet. We ordered the calamari, puttanesca, limoncello cake— highly recommend everything. The pasta dish wasn’t too “big” so maybe order an appetizer! Super homey place. I’ll be back!

The food is always good. No hidden fees or up charges. Family owned , A San Francisco original. Great pasta with a reasonable wine list. Ask for Mario!
After walking through FiDi, Barbary Coast and the entire North Beach looking for just the right place to eat, we finally settled on Caffe Macaroni run by a husband and wife team for over 30 years. This is old school, it’s simple and tiny (we had to sit on the sidewalk). The menu is focused with a solid set of specials and the food was spot on. A fennel orange salad was bright, juicy and salty. A piece of sole over a bed of caponata was perfection. Bread with a garlicky dip was generous. Service was warm and personable and thankfully they were able to provide the tangible version of the menu on request so we didn’t have to view by phone . Prosecco could have been a touch colder for my tastes but all round - this was a dinner that surpassed our expectations and that we thoroughly enjoyed.

The owners truly made the difference. They were friendly, down to earth and good people. The food was authentic and delicious. A must try.
Fantastic. Over from London, wanted something away from usual tourist traps. Neighbourhood restaurant at its best. Service was excellent, food equally good. Very relaxed, made to feel at home. Would highly recommend
The cheesecake was the most amazing thing I have ever tasted. The staff and chefs were incredible in both service and personality.
It was one of the best meals I have ever had. It may have ruined all other cheesecakes for me but I'll not forget that first bite and for that, my life is better

Outstanding, truly authentic old-school Italian cuisine in North Beach district. Highly recommend reservations; inside seating is very limited. Stephanie and Mario are wonderful people and the food was so delicious. Has the Caprese appetizer, fantastic spaghetti & meatball plate, and a delicious lentil soup. Wonderfully convenient location right off Geary & Columbus.

One of the oldest Italian restaurants in SF.
Be patient with Seating and there is a quaint upstairs for events if Covid. Original owners Mario and Stephanie. Mario is the only server.

Mario, Stephanie and the amazing cooks are the best! This is a special little authentic Italian restaurant where Mario and Stephanie consider everyone familia!
What a wonderful welcome and experience in this little sliver of a place! Mario could not be more welcoming or unobtrusively attentive, and the menu is a fascinating collection of Neapolitan dishes labeled in dialect, that are tasty and well prepared and presented.
Lovely and fun unpretentious family restaurant with great food and a convivial service staff.
Caffe Macaroni is a favorite spot for us. We love the vibe — the interior is warm and joyful as are the proprietors. The menu is creative and delicious — the food hits the spot! Great location at the intersection of North Beach and Jackson Square.
